1st Option Home Health & Infusion utilize a telephone monitoring system in order to assist patients and their healthcare providers with their disease states. Telemedicine allows for easy collection of important information, such as vital signs and other data. An alert is sent to our Care Management team in the event that a problem is detected. A member of the Care Management team will first call the patient to assess the problem and if necessary will contact the patient's physician for further direction.

Telemedicine can be used for patients with:
 

• Diabetes

• Cardiac issues

• Hypertension

• Stroke

• After surgery

• CHF

• Respiratory issues

Telemedicine services are NOT used instead of regularly scheduled visits.
